Registration Information
 
1.   Will your event offer different prices for different groups of registrants?
      If yes, what is the number of different price groups?
2.   Will your event offer a discount for early registration?
      If yes, what is the cutoff date for early registration?
3.   Will your event offer a discount for multiple registrants?
      If yes, what is the minimum number of Registrants required to qualify for discount?
4.   What is the maximum number of registrants to accept online?
5.   What is the cut-off date for online registration?
6.   Will you allow refunds?
      What is the cut-off date for refunds?
7.   Will your Event have its own Website?
      What is the URL?
8.   Do you want your registrants to fill out an Event Evaluation?
      What is the cut-off date for the Event Evaluation?
9.   Do you want to request additional information from your registrants during 
      the registration process?
      What is the number of separate questions you would like to ask?

 
Registration Form Questions
 
Designate the price, or prices (up to ten) if more than one price group will be used.
Designate the early registrant discount price, or prices if more than 
one price group will be used.
Designate the multiple registrant discount price, if offered.
Designate the early registration price for multiple registrants, if offered.
 
Questionnaire (Registrants will be asked to check all that apply.)
 
Designate the questions that registrants will need to answer during the 
registration process.  These questions should be presented as statements
that registrants will check if they apply.  Examples are as follows:

   I would like the Fish Dinner.
   I would like the Beef Dinner.

Choose the categories you want your registrants to rate, if an Event Evaluation 
will be requested after the event.

When you complete the registration set-up, you will be given an Event Code and
a link to your Event Page.  You can send this link via e-mail, or direct your
prospective registrants to RegisterToAttend.com and provide the Event Code and
Password (if applicable).
